HOW TO REPORT A CONCERN
If you know or suspect a violation of any federal or state law, regulation, or LISVH policies and procedures, please report it through one of the following:

- Employees can also report directly to their Supervisor, Management, or Senior Staff.
PROHIBITION OF RETALIATION
The Long Island State Veterans Home strictly prohibits any form of intimidation or retaliatory action against any individual or group who, in good faith, reports actual or suspected wrongdoing, including conduct that is illegal, fraudulent, violates LISVH policies or professional standards, or breaches federal, state, or local laws and regulations. The confidentiality of the person making the report will be protected. All complaints will be taken seriously and investigated thoroughly.
INVESTIGATION OF VIOLATIONS
All reports of suspected wrongdoing will be investigated thoroughly by the Compliance Officer and, when necessary, in collaboration with other departments such as Human Resources, HIPAA Privacy Officer, and Legal Counsel.
