Resident's Rights to Return to the Community & Least Restrictive Environment - The Open Doors program
Nursing homes must notify all residents upon admission of their right to live in the most integrated and least restrictive setting. They are expected to refer to the local contact agency whenever the resident and/or designated representative requests to speak to someone about returning to the community.
The Open Doors program is the local contact agency to which nursing homes should refer residents who want to return to the community. Open Doors can be contacted by referring to the online regional listing or by contacting the Transition Center Hotline at 844.545.7108, via fax to 518.465.4625 or via email at secq@ilny.org.
The Open Doors Transition Centers have transition specialists at Independent Living Centers across the state to directly assist people in nursing homes access the services they need to return to the community.
One of the programs available to support individuals in the community is the Nursing Home Transition and Diversion waiver.
